Hi There,

I will sponsor my husband, We are both in Canada now, but we will apply via Buffalo office.

Which Medical form should i use. i found two forms in the website:
1. 3910E-US
APPENDIX C

2. IMM1017 SCL MEDICAL REPORT: SECTION A-EDE/EFE
SPONSOR OR COMMON-LAW PARTNER IN CANADA CLASS

We married at the end of June, 2011, but still waiting for the marriage certificate now in Ontario, more than 8 weeks already. should i waiting for it, or just send record of solemnization of marriage with my other application documents first. and send marriage certificate to Buffalo as soon as i get it later?

For the medical, the designated doctors have the forms in their office. You don't need to bring any with you... just photos. (at least this is how it was here in Ottawa when my wife went for the medical)

As for the marriage certificate, you don't need to wait for it but I would guess that is much better if you include it.
 
Actually a copy of the marriage certificate is the only acceptable proof of a legal marriage. The record of solemnization is not adequate, as it only says that a ceremony has taken place, it does not mean that the marriage has been recorded with the authorized body responsible for registration of these events. Your marriage is legal and recognised in the country where you got married when you receive a certificate from the registrar to that effect. I can think of nowhere that you are allowed to use the record of solemnization as proof of your marriage in a legal context, that is why CIC requires a copy of the certificate.

Bring Appendix C from your Regional Guide and the required number of photographs with you to the medical, the doctor will have the appropriate forms at his office.

Thanks Mercur, My husband call the doctor, she said it's your responsible to decide which form you will use. i will not tell you about this info......... what's happened?

You need to take with you the Appendix C form... http://www.cic.gc.ca/english/pdf/kits/guides/3910e.pdf When you make your appointment with the Designated Medical Practitioner tell them it is for a spousal application for permanent residence. When you arrive you will be given another form to complete... you will be given part of this form to take away with you and include in your application package.
