Medical Form (Inland) Question!

While filling out the pdf form for the inland medical form, I ran across a part that says "Intended Canadian Destination".

1) Would this be where I intend to live?
2) Would it be Ottawa, where the medicals are sent?
3) Or would it be Vegreville Alberta, where the application is being sent?

I would appreciate any help I can receive.
Thanks to anyone how reads this or can give me a helpful answer.

I have the appointment in a couple of days so I would appreciate an immediate response.

when I filled my application, I filled the place I intend to live, for example: "Montreal Quebec"
To me only this makes sence. Where the medicals are sent u dont care, the DMP will take care of that.

Yes, this is the intended destination where you would live if granted PR. So if you plan to live in Toronto, you would say Toronto, ON
