Medical form IMM 5743

When completing form IMM 5743 (to bring with us to medicals), there are two sections:

CLIENT CONSENT (client 16 years and over)
and
DECLARATION OF PARENT/GUARDIAN (client under 16 years of age)

When completing this form for my kids, I initially thought to only complete the second section, but then it misses out information like Date of Birth, and the actual paragraphs we are supposedly agreeing with, so I'm now thinking that for kids I should complete both sections.

Does that sound right? What have others done?

We did not fill any such form... :

As far as I remember, I signed one consent page given to me by the doctor which was in a different format...i just signed mine and for my kids and separate pages...

Thanks! Interesting that it isn't the same procedure. We were sent the IMM 5743 forms by the medical practice when we booked the appointment, but it is evidently a CIC form as they seem to be downloadable also.

I'm currently thinking to fill in all permutations and see which they want when I get there!

In Jeddah, Saudi Arabia (link: http://www.cic.gc.ca/pp-md/pp-list.aspx), it seems that there is only the following Panel Physician. Are there any other ones also?:

Also, should the 4 photos required be latest?
