tell the doctor that you are doing the outland procedure, and when asked, tell him it's Sao Paulo office. He will give you a paper to confirm that you have done the medicals, and you add that to your application. You don't get the results, they are sent directly to Sao Paulo. You don't really get a confirmation directly, but once you have sent everything to Mississauga, and got the sponsorship approval you will be able to check your status online, and you might have a line that reads "medicals have been received". nothing else...

I think this is correct. There is a central office in Ottawa that processes all the medicals no matter which country, and they do the actual analysis of medicals and write up the summary of results that is then sent to your VO which would be in San Paulo. So i wouldn't worry here.
It's all done electronically so should be very fast.
yes, it's OK, it's not a problem. Your medicals results will arrive at the VO before your file, and they "wait patiently" there! and they are matched up at some point ( it used to be when the file arrives at the VO but now with the procedure it can be done even before).
I did my medicals in February 2012 and sent my file in April. Sponsorship approval was 3 months at that time, so my file got to London VO in July - and it was not a problem. So a few weeks difference is not an issue! just make sure that you get the paper from the doctor to add to your application so it can be matched up.
