Spousal medical, you go to the DMP, they send the results to the regional office.
You get a Section A IMM1017 document that you include with your application.
The medical results will be requested (when they need them) by the visa office that you apply to.

The medical is usually valid for 12 months, police checks typically 3 months.

No, the medical is still valid for 1 year from the date it was completed. You could use it for any immigration application done within that time frame.
