Hi all,

I have my medical examination tomorrow, and just want to touch base with everyone to make sure I have everything in-order.

I plan on taking:

*My passport
*My glasses
*I have no previous medical tests so I can't take anything in-terms of this

Here is the bit that I am confused with - The CIC website states I need to take the Medical Report form (IMM 1017E), however, I can not find this form anywhere. In addition, I read that the panel physician provides the IMM107E form, is this true?

Lastly, the test is being conducted by the following:

I forgot to ask over the phone if I need to bring four recent photographs, and now they are closed. Has anyone been to the above physician that can tell me if he works with the eMedical electronic system?

According to the Panel Physicians list this doctor is recognized by CIC.
Bring 1 or two photos for ID. One is required for the e-medical.
As for the IMM107E it is not needed as you will be having an UPFRONT medical be sure the physician knows you are there for that purpose.

The place in Ottawa where I did mine took the photos in the office. All I had to bring was myself and my passport (and of course the hefty sum of cash). Not sure if this helps you or not. How early is your appointment? Could you call them in the morning?
