I'm a little nervous about my medical exam that I want to get done for my spousal sponsorship...
But I have a few questions first.

One I don't have health insurance. Do I need it?
Two I need to know the cost. I heard that it is expensive.
Three how difficult is it to get an appointment?

the exam itself is nothing special> medical examination, urine and blood test (depending), and X ray.
I'm not sure that you will get an answer about a physician in particular.
Call the doctor's office, and ask: what is the cost, do I need insurance, and how soon can I get an appointment.
You'll probably will have a better and faster answer contacting him directly.

I don't see why insurance would be needed. No matter where you are the medical test is usually paid for on the date of visit and has nothing to do with insurance.

As for price, this varies by doctor and location. In a place where there are many CIC panel doctors (like Toronto area), prices are cheaper due to lots of competition so the average price is around $150. However in areas where there is only 1 doctor to choose from, they tend to have higher prices since they know there are no other alternatives for the applicants. I've seen prices range anywhere up to $400 or so for all the tests.

As Sweden said, the very best advice is just to call the docs office and ask.
