I am taking my medical exam tomorrow for PR status - but we have not submitted our application package yet. I'm being sponsored as a conjugal partner. I do not have a form 1017. My sponsor in Canada called the CIC help line, and they said this wasn't a problem. I emailed CIC from here three weeks ago with the same question - never heard back from them.
It still doesn't make sense to me that I can take the physical without having first applied and received the 1017 from CIC. Am I right to worry?
By the way - there's only one doctor in the SF Bay Area approved for this - and he's charging me $450. In LA it's $300, and in Canada (Montreal) it's $200. What a racket this bay area doctor has going.
 
You're worrying needlessly. The 1017 is provided by the doctor once your medical examination has been completed.

As for the prices - different doctors charge different amounts. As a general rule, it's much cheaper in Canada than it is elsewhere. My husband paid $175 in Toronto.
