If I apply outland while living in Canada, I presume I have to get my medical done within Canada.
Which form do I use to take to my medical exam?
 
In Country Specific Guide, Appendix C.

Make sure to check the latest DMP list on the CIC website and call ahead to make appointment (some requires) and confirm the number of photos to bring in.

I have another question if you know, when I apply outland, what do I use as my current resident address? I am in Canada and have been since July 2011.
 
If you were granted status for 12 months or longer, you can list your Canadian address.
