Medical exam paper

When compiling your paperwork, which paper do you need to have when it comes to the medical? Do you just need the piece of paper saying you did in fact go get one? Which one is it again? I'm making double-y sure as I get ready to send mine off.

it's the "emedical" receipt the applicant receives after the exam. it has the applicant's picture, personal info and passport info on it. make sure to keep a copy for yourself.
