Just noticed that on the Document Checklist IMM 5491, it says that the "original copy 2 of form IMM 1017 signed by the examing doctor"

What do they mean? The Dr office gave me a photocopy of IMM 1017 Medical Report Section A, Client Identification & Summary....not the original...IDK what "copy 2" means.....will this be a problem?

Also, the Dr had to run to a funeral, so the gal at the office stamped his signature. Is this a problem with CIC?

AND they assumed I was applying Inland and were surprised when I told them I was applying Outland because as a US citizen I get the choice. From what I've read, the medicals can't be sent to the wrong place. I should be good, right?

He was suppose to give you either the white or green copy i don't exactly remember with a picture on it and all the information properly written on it, as for the stamp as long as his license number is on it you should be fine. It's a carbon copy that we receive
 
The doctors office should have given you an appendix C it is actually carbon copied triplicate each copy gets a photo attached top copy goes with the medical exam 2nd copy to you and in with your application and the 3rd copy goes with your x-rays... just realized you are applying inland so it might be different... maybe the doctor you went with doesn't have the newer forms... the nurse did mention when I handed her my appendix C (that I had printed online) that they had new forms CIC had issued to them. but after looking at the form you are talking about it looks exactly the same. ::)

The Document Checklist specifically says Medical Report-Section A, (which is what I have) not Appendix C. It does say on the upper right corner, "PROTECTED WHEN COMPLETED-B" , so maybe it is the second copy...but it's a photocopy...... ???

I don't remember there being a colored triplicate form, but it did have my picture stapeled to it. The form I took to the Radiology and Lab I think had multiples because they each took a form.

The Medical form they're using is dated 05/1998 at the bottom. OMG. I need to call the office!

I am applying Outland, btw.
 
Just spoke to the Dr office gal in charge of sending the Medicals to CIC. She said that they don't get/have triplicate Section A forms. CIC sends them a CD with the forms on it and tell them to make copies from it.

The Appendix C that I provided, was mailed with my original Section A that the Dr sent.

So, tho IMM 3999 and the Document checklist state that copy 2 is to be sent, IDK what to do. Makes me wonder if the Dr office isn't up to par and should I notify CIC?
 
Did you tell the doctor's office that you were applying out-land?
cus the form you are talking about says right at the top "Medical Report: Section A EDE/EFE Spouse or Common-Law Partner in Canada. This is why when I replied before I assumed you were applying in-land ... I never provided this form with my application. sorry I am of no help but my best bet would be its because you went to an "in-land" doctor...

Edited to add: something else I just noticed on that IMM1017 upper right hand corner it says "Case Processing Centre - Vegreville" again this is in-land and is your medical going to go there instead of Miss. or Ottawa?
