My question relates to Medical Exam form Annex C:

It ask on form to indicate processing center and well not certain what to put in there.

As I understand it's processed in CPC Mississauga and then sent to Moscow.

Whats needs to be present here?

Hello Yan,

you need to indicate Moscow there. Once you complete the medical visit, the DMP will give you one copy of the medical exam ( not the actual results but just a paper which shows that you have done the medical) and you need to include that in your application.

THe results go straight to Moscow and "wait" for your file there. You send your file to Mississauga, and once they have approved the sponsorship part, they will send your file to Moscow where your medical results will be matched to it - hence the importance of having the paper copy from the DMP.

Medical results are sent to a regional medical centre, where they are evaluated and entered into the Immigration Medical System. Only the evaluation code is transferred (manually) from IMS into GCMS.

The IMM 1017 EDE copy that you are given has a unique serial number. This allows them to match up your results with the results in the IMS computer (since the medicals are specific to your type of application and should not be used for other types of applications.)
