Medical Exam in Russia before application or after application

Good day,

I am sponsoring my wife.

Is Medical Exam in Russia before application or after application is submitted?

I was told that Immigration Canada will ask her to go to doctor after processing the application to sponsor.

I tough this with the application the MED exam by doctor before its sent in.

Can someone clarify to me which is right?

It is a standard pocedure even for Moscow visa office. Medical exam has to be done before an application is submitted.
