Medical exam for visitor

My mom got an approval for extension of Visit but asked to do Medical exam until 29th Jan 2018. She's leaving on 30th Jan. I know she has to go through the Medical but can she do it at her home country with a Panel Physician? She will be comfortable communicating in her own language with a doctor that's why she preferred to have the medical back home.

If she is already out of the country there is no need for the exam.

I am having same issue. My parents extension is approved and medical is asked withn 30 days but they are leaving in 1 week. Medical costs $250 and it is expensive. Pl advise what I should do?
