Hi everyone,

I'm bringing my wife for her medical examination on Wednesday. I just wanted to be sure we have all the required documents. We have her ID (passport), photos, glasses, and previous medical information. However, we also saw a reference to a "Medical Report Form (IMM1017E), if you do not undergo an up-front medical examination". (http://www.cic.gc.ca/english/information/medical/medexams-perm.asp). The site said that the visa office would mail us this form, but I have no idea when this was supposed to happen. Should I be printing and filling out this form now?

Also, will they ask for anything else? She's doing an outland PR application from the US.

You don't need the IMM1017. As for anything else they may ask for, you need to contact the medical office to find that out; beyond the passport, there is no set list of requirements. Some need photos, some take their own photos. Some ask for medical records, some don't. Different offices, different countries, different requirements etc.
