Hi , I would like your help , I came in Canda on January 2018 , I did the medical exam on that time , and now I need to extend my study permit , so they are asking me to submit my medical exam again , I have done my medical exam and I can use that one because it hasn't been 12 months yet , not it's OCTOBER THIRD 2018 , I have E-MEDICAL FORM SHEET , but I don't know which documents I should submit .
Should I submit this document that says :
E-medical
Information Sheet
It's my photo on the right top corner and the IME
