Everything which could cause too high costs to the Canadian health care system as far as I know. Or infectious diseases.
 
Tuberculosis, HIV, Syphillis are for sure.

Heart murmurs, heart defects, infectious diseases like hep A,B,Cs, HIV, syphilis, past or present TB, liver disease needing transplants, blindness, deafness, certain degree of physical disability as it require additional care from healthcare system. Also need of dialysis. That's why they do blood and urine test.

Not one of this would be cause for instant rejection. Even people wit active HIV were known to get pr.
It depends on your general health, if you are likely too need serious medical attention in the next 10 years etc.

So dont freak people out for no reason
