Medical checkup

Hello.

I have a open work permit till July 07-2018.Before coming to Canada me and my family has undergone medical checkup examination now i'm interested to pursue my higher education.

I have received the acceptance letter from the college i need to apply for study permit.When i'm applying for study permit CIC portal ask for medical proof reports.

As the medical reports where directly submitted to the Canadian embassy i don't have the reports how to upload the details on the portal.

Suggestion please

Based on my experience, the doctor gave me a receipt with an IME barcode as proof of taking the medical exam. She said that since it's valid for 1 year, I can present this form anytime I am asked for medical proof reports. Do you remember receiving some sort of receipt from the doctor back then?

we are in Canada right now and the receipt is in India.Any alternate ways

Did the embassy ask you to upload the medical report online? If yes, you can ask somebody from your home country to send a scanned copy of your report so you can upload it in the online portal.

I don't have any family name mentioned on the passport.When i'm trying to fill the form it's asking for family name what should i do ?
