Mailing in application for visitor extension--2 questions

We are very close to submitting spousal application and meanwhile I am requesting an extension to stay in Canada while waiting for the process.

As I am getting ready to mail it in I run into 2 issues that I am hoping someone can clarify:
1. do we need to send in pictures for a request to extend stay as visitor?
2. I have paid the fee online and printed out a receipt and I am not sure if I should just sent it as is or if I should fill in my name, address, birth date and phone as this sections are blank ... Please advise
