Hi everyone! just wanna ask your opinion on what should i use on my forms. some facts:
january 2011 - became permanent resident. still single at this time, thus, PR card reflects my maiden name
june 2011- got married

so now my hubby and i are filling up our forms and was wondering whether i shld still use my maiden name on my sponsorship forms so that it will coincide with pr card (and passport) or do i need to use my married name already?

another question: i noticed that the forms only ask for last name and given name. should we include middle name in the given name? bec if not, will this not be a problem for me because when we ( my mom dad brother and i) applied for PR under skilled worker (dad's) we included middle name in the given name such as our PR cards read LAST, GIVEN MIDDLE ?

would really appreciate all the help i can get :) thank u!
 
Use the name that matches your ID.

First and middle names ARE given names.
