You need to contact the Islamabad Embassy. I have no idea how an application can be processed to completion without having a full name for the primary applicant.

If they are Muslim (assuming, sorry if wrong) the fathers name is the second name. So, the daughter of part should have a name after it, that is the last name.

For example:

Sarah's father is Tom, her name then would be: Sarah Tom

She didn't marry her father she married a man that should have his own last name. Do Muslim women not take their husband's name at marriage?

if she carry pakistani passport then it has option of last name whts there,,and if there is no last name on her passport then she can use her husband name or father name which she has on her passport,,and she can also travel with only one name ,,,i have my passport with only single name before marriage and i m also from pakistan

I based my reply from my own experience. It is not common that a wife takes the last name of the husband. I am also Muslim and I did take my husbands name. However, the majority of people I know have not.

I have heard several reasons for this, namely that it is haram (bad) to take the name of someone that is not your father. Also, because the name of the father is taken on each generation thus keeping a family history of the fathers side.

An example of this is: Sarah has a baby, she names the baby Frank. Her husbands name is William, his fathers name is Kevin, his fathers name is Ronald.

Sarah's husbands name is: William Kevin Ronald
The baby is thus named: Frank William Kevin Ronald

When Frank has a baby he OR she will have the names added to what name Frank and his wife have picked. Example: Tammy Frank William Kevin Ronald

Take Care
