I would not include the E-mail if I were you. Although it's not rude, it's just unnecessary.
And for your second concern, you can do both on the reference letter, hourly and salary. This is what my company did. Mentioning like Mr.XX was employed for $XX.XX per hour and he made XX.XX at the end. (Something along those lines), never hurts to mention both.
