Two passport sized photographs
Set of fingerprints
Copy of 1st & 2nd pages of passport
Return Pre-paid Fedex slip
Return address information
1. You can go to the nearest Police Station and request that a set of his fingerprints be taken
2. Get a bank draft for to equivalent of JA$8,000, for one day service, or $6,000 for five day service. Draft to be made payable to The Commissioner of Police, Jamaica
3. Fedex to : Technical Services Division - 34, Duke Street, Kingston, Jamaica. Their phone number is (876)922-0125
4. Package to be dropped off at Full Service Fedex office and pay for sending & returning, the return pre-paid slip is to be put inside the envelope.
