Issues in printout of the IMM1294E form

Hi I filled the form IMM1294E and took printout but there are some issues like the fields are not correctly showing having drop-down options..

Fields which are showing errors in printouts includes:
1. Citizenship (It displays number 209 instead of name of the country)
2. Your current marital status (It displays 02 instead of single)
3. Native Language
4. Country of Issue
5. Complete address of school in Canada (Province and City/Town)

Can anyone please share some solution..

did u validate the form. I guess there is a problem with your adobe acrobate reader. update it and check

Yes I validate it.. after validation page 5 also appeared.. but it prints only four pages... I am using adobe reader 9..

I don't know what could cause the problem. Use some other computer. pages should be 5. I did it three days ago and gave me 5 pages. Go to a composer shop or an internet cafe they have printers there .
