Is there a possibility of being rejected after passing the medical requirements

Good day just want to ask what is the possibility of being rejected after a medical, My wife is a skilled worker and i am applying for Spousal Open Work permit, My two sons ,Student fOr my 8 year old and a visit visa for my 3 years old. Im worried because the PIASI call center advised me that we dont have to submit IMM 5257 shed 1 because weve answered " NO" to background info but thru reading comments, some say it is needed others Said if you say NO you dont have to submit, can i go this far of the process if they have noticed that may application is incomplete. Please help, lalo na sa mga kababayans na FILIPINO baka may same expirience po ng sakin na hindi nag submit ng Application for Temporary Resident Visa Shed 1 IMM 5257. SALAMAT

Here are the dates of my application form the time i have submitted our application
May 22, 2013 submitted thru mail
Aug. 15, 2013 got the medical request
Aug. 30, 2013 finished the medical
Sept 18, 2013 passed by the doctor to immigration
