IQAS help with form

Hi there!

Has anyone used IQAS before and could give me some advice?

I want to get my education evaluated by them, but when looking at their form I'm not sure about the documents that are needed. Apparently I'm not allowed to post a screenshot, so this is what it says:

Photocopy of the Degree/Diploma (name in the original language): | Photocopy Enclosed
Photocopy of transcripts/statement of marks: | Photocopy Enclosed
Transcripts (mark sheets, grade reports) issued by: | Transcripts Requested from the issuing body
Degree/Diploma (name in the original language): | Photocopy Enclosed
Photocopy of transcripts/statement of marks: | Photocopy Enclosed
Transcripts (mark sheets, grade reports) issued by: | Transcripts Requested from the issuing body

First three lines make sense to me, but the three after... what are those for? Looks just like a duplicate from the above, why do you need it two times? Is the second part optional? Or what's the difference between "Photocopy of the Degree/Diploma" and "Degree/Diploma" since both say "photocopy enclosed"?

Did anyone add additional information at the bottom and if yes, what did you write?
