Inland application. check list form..

Hopefully someone can answer this question this time..


We are in the last stage of our application and hope to send it in tomorrow,
but there is still few things confusing us. Do my husband and I should complete each a check list form
or just one for the two of us? Also, should we put N/A when something in the check list does not apply to us or just leave it
empty?
