No - you don't have to present a new application. However if your baby is born in your home country then you will have to add your baby to your immigration application (after it's born).

Being pregnant makes no difference to your application. This also won't speed up the processing of your application.

If you haven't had your medical yet then you won't be able to immigrate to Canada before the baby is born. As part of the medical you must have x-rays and these are harmful to newborn babies. So if you are asked to do a medical, while you are still pregnant, you should do the entire medical apart from the x-ray and then notify CIC that you are pregnant and won't be able to do the x-ray portion of the medical until after you have given birth.

If you want to deliver your baby in Canada before you become a PR then you will need to apply for a visitor visa. Note that there is no guarantee the visa will be approved. Also, you will be responsible for paying all costs related to your pregnancy and labour once in Canada. You should budget a minimum of $10K for the delivery just in case there are minor complications.
