India PCC - Period of Stay

Hello Members,

I am Indian Citizen and am working in Canada since last 3 years before moving from India.
I have to apply for PCC for immigration purpose and have queries about "Period of Stay In India" on "Police Clearance Reference" Form to be submitted in 3 copies.

Queries:
1. What is expected to fill in single line in "Period of Stay In India" ? Is it number of days or actual dates?
2. Do I have to fill all days or dates since my birth or for last 10 years ?
