Hello, I am trying to fill out form IMM 5481 where you prove your eligibility.
It asks you to choose between "unemployed", "employed", and "self-employed"
As an independent contractor, I would assume that I am self-employed, but that section only leaves space for "name of business" and date your business was opened. Should I choose unemployed and attach a page explaining how I make money, or should I choose self-employed, write N/A where it asks for business name, and attach a page explaining what I do?

Don't choose "unemployed". Choose the other and include a letter of explanation.
 
If your a contractor do you work for the same company every day . I am self employed too but work mostly for the same company , so i had them write up a letter for me saying how long i had been with them and roughly how much they pay me each year.
