IMM5708 - Extending stay for the 2nd time (Visitor's record)

Hi !

I'm stuck on the field n. 7: Current country or territory of residence.

I've come to Canada on Dec/2019 and I've extended my stay for the 1st time from Mar/2020 to late Jun/2020. Now that I want to make my extension again, which date should I put in the From value? Dec/2020 or Mar/2020?

Yes I have, question basically asks you since when have you been in Canada. Plus include copy of your old visit visa document

So do you mean the old document they gave me at the boarder or the IMM5708 I've made for my first extension?

Yes. The more documents you provide, more easier it is for the immigration officer to deal with your application
