I get that you have to list all addresses I have lived since 18 or 10 years which is most recent. For me, I'm 25, so, since 18 is my most recent.

What date would I put for my present address? I have lived here since February 2012, so I put 2012/02 for the "from" what do I put as the "to"?

yes. or if it allows, i suspec n/a would also be sufficient

You can put in the date that you sign the application as well. That will be acceptable and how many folks proceed with that type of thing.
