Hi all... does this form need to be filled out by both the sponsor and the applicant?

Only the applicant.

Follow the application guides 3900 and 3999.

Thanks - I had been following the guide, but was thrown off by this:

This form must be completed by:

you, the principal applicant;
your spouse or common-law partner (whether accompanying you to Canada or not), and
your dependent children aged 18 years or older (whether accompanying you to Canada or not).

You will see the same thing on IMM5406.
That happens because it's generic forms, used for other application categories as well.
