Hello,

Filling out this form gives alot of questions.
1)How are you doing when there is not enough space and additional info is on separate sheet. Do You start writing in the box first and then finish on separate sheet? Or is it better to write in the box: "additional information attached" and the whole story put on that sheet? (as for ex.question 11 where you have to give names of your family members when sponsor met them)

2) question 27. gives only blank boxes for dates, but I want to attach additional info, where do I mention this on the form, so that they can look for the full answer?

Apparently there will be many sheets of paper attcahed to this form, should I paperclip them all to this form?

I will appreciate if you can share your ideas
 
When I filled this form in, I put approximately one paragraph in the box and then directed them to the seperate sheet of paper and yes I had loads of additional pieces of paper for the form and I believe it says not to staple the pages together and I thought there were too many for paperclips, so I got some little bulldog clips and held them together that way.

What i did was fill on the available space and wrote on the last box "refer to attached sheet for additional info".

At the end of the questionnaire you are given the opportunity to write and attach any additional info. If i were you i would use that portion to further clarify whatever you wished to add. It wont even hurt to quote the question and then elaborate.

Make sure you write the form and question number on top of every sheet attached.

I wrote "Please see attached page" in any blank that had an answer that would not fit in the blank. Then I typed the entire answer on a separate page (labeled with IMM 5490, our names, and the question number). I put each of those additional pages behind the page of the relevant question. I also had many extra pages of supplemental information (photos, bills, bank statements, letters, etc, etc) that did not refer to a specific question but did offer support/"proof" that our relationship is authentic and continuing (as requested at the end of IMM 5490). This information was in a separate stack, but also labeled with IMM 5490, our names, and "Supplemental Information."
