there's a note says: If this application form is completed electronically, place the barcode page (Page 5 of 5) on the top of each individual application package

Placing the barcode page on each individual package: Does it mean that I have to print the barcode page according to the number of the items that I will fill and enclose, and then place it on the the top of each item?????

No, you just have to print that bar code once and place it on top of the application form IMM1294.
