IMM5475 - Authority to release personal information to a designated indivisual

Hi guys,

I am sponsoring my spouse and i am a Canadian citizen.

Does my spouse need to fill out the IMM 5476 and list me as a representative right? if she is 18 years or older?

and

I am confused with IMM5475. Does my spose need to fill out this form and submit?

It is asking designated individual name and i dont know what that it...
