Hi guys,
So I was wondering for my express entry application I am supposed to provide IMM5409. I had already filled out this form in July 2019 for my work permit extension and declared my common-law relationship. Am I required to fill out a new form for my express entry application?

Yes if you are in a common law relationship
What you submitted in your work permit application is irrelevant
