Hello,

How to fill out IMM5406 Additional Family Information i.e. each family member (has to complete this form) must put his name under field entitled Applicant,correct? My concern is that applicant in IMM5406 does not relate to Principal applicant (me) but to family member who completes this form (wife,child)

you and your spouse will need to fill this form. you will have to send two copies.

if you are the principal applicant. then in 1st copy you must mention yourself as the principal applicant and your wife as spouse

filling form by your spouse, in this case your spouse will mention herself as the principal applicant. and you as spouse.

Please is form 5409 compulsory for all applicants with spouses? If yes must we fill the Commissioner of oaths section? We already been married for 10 years with a certificate to show for it.

it's statutory declaration of common-law union, if you are legally married and you have a marriage certificate you dont need to send this form.
