Hi,

I'm trying to fill in this form on my laptop. I am the Spouse and Sponsor for my wife, do I need to complete this form also?

As well, in the "Marital Status" space it does not give me enough characters to write "married - physically present". Am I doing something wrong?

Also, my wife's father is deceased. It also does not allow me write this word.

Any help would be greatly appreciated.
Thanks

Only the Principal Applicant fills the form. PA can hand-write the words later after printing the form. Just put the word "deceased" under 'Present address' column.

About your status write only married if u dont have space to write physicially present you cab add a separate paper where you can say that you were both present

For my mom, I entered Deceased under present address.

Not that I remotely recommend doing this at all, as I can tell you I've done nothing but think about in the three months since I realized with horror I did so, but after printing out the form I ended up forgetting to write in married - physically present. I also forgot to write in my present address because it, too, was too long to fit. I had intended on writing them in but somehow that got skipped over when we were putting the package together. I've never been asked for an updated copy of the form, and my background check just started recently, so I doubt I'm going to be asked for it at this point.

My advice: Anything where you're going to have to hand write details in after printing, keep a list and go through it to make sure you hit all those spots before mailing off your application. There were a few forms where there wasn't enough space to type stuff properly, at least for the information my husband and I needed to enter. I managed to get all of them except the ones on that particular form.
