This .pdf file is set up with small fonts in the upper half of the:
Name, Place of Birth, Marital status, and Present Address fields.
Why would they do this if they are requesting your native language just for the 'Names' field?
Have they made an error in formatting, or do they really want the other fields filled out in two languages?
