IMM5257 - Who should fill this out?

I am applying for a student visa and I am asked to sign imm5257. I am applying alone but I have a spouse and a daughter, both are NOT accompanying me in Canada. Does all of us need to fill out the form or is this form just form me ?

This is the form for TRV. Only the person travelling needs to fill it.
