IMM1295 Application (HELP)

Currently having problems with my IMM1295 Application and was hoping somebody could provide me with a step-by-step process.

So I download the form, open it using Adobe Acrobat, complete it as best as I can, using 'not applicable' where needed and I get to the end of the form.

Now comes the part where I need the signature and date, how do I go about this? From what I can gather I need a digital signature, how do I create one and then how do I add it to the PDF and get it verified?

You have to print it once you complete filling it and than sign it by hand and send it to concerned authorities !
