IMM1294E won't generate barcode

:'(I finally managed downloading the PDF from that"Please Wait..." browser page, and open up with the Most Up-to-date Adobe Reader DC.
The issue comes after I fill out the form, click validate, and the PDF just simply won't generate any bar code, I tried turn on the Javascript in Adobe, tried using Acrobat... can't get it to work. Could someone advise/help me to get this thing working? or send me a new form: adamhezhu@gmail.com

I have a macbook and that problem happens with me too, but i sent the same exact file to a different computer running windows and it was showing barcodes on that. Its just not supported on macbook, so just click validate and leave the barcodes if you're applying online.
