IMM0008 wont display / error message

There is a message about Adobe and that my browser cant open it. My husband had the same issue. He has a Windows laptop and I have Mac. And we have both updated to newest version of Adobe. I think the form online is corrupted or something. Can someone please link me where I can get this somewhere else online?

Instead of trying to open it in your browser, go to this page and right click on the file and click Save Target As or Save Link As. Once it is saved locally to your computer you will be able to edit it.
