Imm for wrong

I did my medical exam as required but i got the form before anything was done so it says required on all parts. They are e medical aproved doctors office so i asoume they send it right. By my form as it is now is useless....how do i get a correct filled form without "required" on it?

It should be OK because it will be electronically updated and linked to your medical number, but if you really want a new form you ask the clinic that did your exam for a new one. Did you ever try asking them?
