IMM 5669 asked for, not sure who should fill it in.. HELP!

Hi guys,

I have had an e-mail come through asking for me the principal applicant to fill in the IMM 5669 form.

However, my common law partner who’s visa is linked to mine has not been mentioned in the e-mail. Does she have to send one also?

Any help would be greatly appreciated... thanks!
