IMM 5669,5406 does an accompanying dependent 15 year old need their own ?

I have been requested to send in a form IMM 5669 and IMM 5406for my stepdaughter. I have sponsored her along with my wife, from the USA. Now, on the form IMM5669 at top,before question #1. being she is 15 and my wife is the principal applicant I thought, what box do I check on the daughters form? She is a dependent child, 15 years old, and if the form is separate for her, do I just check Principal applicant, is this her own application, or what? And on the IMM5406, section A, do we put her as the Applicant, and fill in her father and mothers name and info as well then?

Form IMM 5669 : your wife will check the box Principal applicant. Not to be filled by children below 18 years
Only dependent children aged 18 or over (whether accompanying to Canada or not ) need to complete the forms individually by themselves.

5409 Section A is for Principal applicant -Your wife's details

Section B -Details of your step daughter.

I am on permanent resident (PR) and my wife and four children are also on PR status.However,My new born baby who is born outside is in Canada on visit visa status.
I need to fill IMM5669 form .My question is that who will be the main applicant i.e My self or my new born baby and all the details to be filled for me or for my new baby.
