IMM 5540 employment history

how accurate does it have to be?
except for current job, I have no clue when I started/ended all the jobs i had in past 5 years
I know roughly what year or maybe which month I started/ended but to which day?
I bounced around alot too lol
and, I've always been paid per hour, so can I put hourly wages? or put rough estimate of monthly income?

You need to put all the details that you can. Some people order a copy of their employment history from the social security office in order to have the correct information.

ok... and for wages... I can't put monthly wage since it's different every month

If it were me I would put a monthly estimate but someone else may know better. I think they just want a general idea of what you were earning.
