my wife is going to sponse me to apply immigration. But she has two part-time jobs (server and sales) right now. so which one should we put in the evaluation form (IMM5481)? or I should print two copies, one for each job?

If it was me, I would include the job that has the highest income. Then attach a note that there is a second job that she is working. Give the same details on a separate blank sheet.

Basically, it's not really going to matter too much anyway. They are only going to be looking to make sure she isn't collecting social assistance, since spousal sponsorships are exempt from Low Income CutOff (LICO).

you mean if my wife annual salary is lower than $20,000, she will not be available for sponsor? and I have to go through other program?

No. I mean spousal sponsorships are exempt from that. She just can't be bankrupt or receiving government assistance.

I agree with this. Do not print two separate forms, as that will most likely only confuse CIC. Instead, put one job on the form, then attach a separate sheet of paper where she explains that she has a second job, and give the details of that job there.
