Ok, I have one answer, now another thing here on Question #7.
I have the "From date" and will put in "Present" after printing blank in "To Date", but as far as "Monthly Salary/Income at the end of that line, my wage varies up and down each pay period over 2 weeks each month.So, should one take an average of the years Net earnings per month, or are we using Gross earnings, average for the month?

its not a big issue, they just want to confirm, if you are employed or not :) you can take an average of the years/ month
 
Ok, but with my calculation on Net income on a the previous form 5481, which starts back 12 months from today, just divide that net by 12 months should suffice?
I just didnt want to put in a huge gross amount, when maybe they are looking for Net instead.

just put in your gross annual amount.....net is too complicated and variable....
